CNAP Registration Instructions
Click on the link below to open up the CNAP Registration Form.
PC Users: Right click on this form and select "Save As" to download the form to your computer.
Once you've downloaded the form, print it out, fill it out and either
1) Fax it to BWK Rogers PC at 612-332-7060 with "Attn: CNAP Registration" on the cover sheet.
2) Scan and email it to: cnap@bwkrogers.com
3) Mail it to:
